The Department of Housing and Urban Development (HUD) has mandated that each HUD recognized Continuum of Care (COC) must implement  a Coordinated Entry System effective January 1, 2016.  The Brown County Homeless and Housing Coalition is part of the HUD recognized COC, called The Wisconsin Balance of State COC.

Coordinated Entry:
1) is a process to coordinate program participant intake, assessment, and provision of referrals
2) covers a geographic area
3) is easily accessed by individuals and families seeking housing or services
4) is widely advertised in the community
5) includes a comprehensive and standardized assessment tool.

All of the HUD recognized COC’s in Wisconsin (Dane, Racine, Milwaukee and the Balance of State) have chosen the Vulnerability Index-Service Prioritization and Decision Assistance Tool (VI-SPDAT). For more information on this tool, please visit OrgCode Consulting, Inc

To find the client rights and responsibilities form and the agency / staff agreement forms that are required, go to and look under implementation.  If a client is entering your program by Coordinated Entry system, these forms are required.
